What is GAPP?
GAPP (Georgia Pediatric Program) is a Medicaid program that provides skilled nursing and personal care services to medically fragile children under the age of 21 AT NO COST to the family. These services are provided in the comfort and safety of the child’s home and community. Families who are eligible for Medicaid can apply for GAPP and approval is based on the child’s medical needs.

Pediatric Rehabilitation

Life-Changing Medicaid Home Care Program for Children Under 21
Through our Medicaid home care program, we offer healthcare services at no cost to children under 21 who have complex medical needs. This program provides ongoing care and support in the child’s home, ensuring they receive the care they need to thrive without the stress of hospital visits.

Intake Process

Two
A registered nurse will come to your home to complete an assessment of medications and the therapeutic care needed and develop a schedule that reflects your child’s daily routine to ensure continuity and consistency.

This program is not a waiver service, which will allow your child to receive services immediately following service approval.

Three
Upon receiving the approval for services, a registered nurse will accompany the nurse, personal care assistant, or companion sitter assigned to your home to ensure that communication, efficiency, and professionalism at the start of care.
